Print and sign the completed ACH application

1. Print the completed form.

2. Sign the form.

Both principal officers specified in Section 4 must sign the form.

3. Fax the form to PayPal at 1-650-237-8886 with the following:

• A copy of both sides of both principal officers' driver's licenses.

• A copy of a voided check from your business bank account

• Copies of your three most recent monthly bank statements.

4. Click Continue to complete the enrollment process.
